As Senior Director of Care Redesign, Crystal Eubanks advances strategy and program design and execution, partner engagement and business development within PBGH’s Care Redesign and California Quality Collaborative programs. She also leads CQC’s technical body of work for behavioral health integration and guides technical assistance for provider organizations participating in CQC improvement collaboratives. Prior to this role, Crystal advised and trained health care delivery organizations, their improvement teams and practice coaches in CQC’s Practice Transformation Initiative from 2016 to 2019.

Crystal brings quality improvement, clinic operations and leadership experience from on-the-ground work with ambulatory/primary care delivery systems and Medicaid and Medicare populations, along with a passion for transforming chronic disease and behavioral health care through data- and value-driven innovation. She has served as quality improvement director, coach, and consultant for various community health centers and consortia, health care Managed Services Organizations and public health departments. As Director of Quality and Data Analytics at LifeLong Medical Care, Crystal led care transformation and improvement initiatives to achieve multi-site recognition as an NCQA Level 3 Patient-Centered Medical Home and national recognition as a top performing health center, as well as the integration of behavioral health.

Crystal earned a Master of Science in Healthcare Quality from the George Washington University School of Medicine and Health Sciences and a Bachelor’s degree in International Development from the George Washington University.
